UnitedHealthcare Community Plan provides a state-specific health plan for certain Medicaid recipients who live in the state of Arizona called UnitedHealthcare Community Plan. UnitedHealthcare Community Plan is a long-term-care health plan for physically disabled adults and children.

In order to be a member of UnitedHealthcare Community Plan, you must be enrolled in the Arizona Long Term Care System (ALTCS), which is part of the Arizona Health Care Cost Containment System (AHCCCS).
